POWO is fairly outdated on cactus taxonomy. Cephalocereus multiareolatus is considered in recent publications (https://www.researchgate.net/publication/322291955_Phylogeny_and_Circumscription_of_Cephalocereus_Cactaceae_Based_on_Molecular_and_Morphological_Evidence) to be a valid taxon, separate from C. mezcalaensis.
